fix(icons): swap FA4-only class names for FA5 equivalents

Browse Source 
fa-clock-o/fa-folder-open-o did not exist in the bundled FA5 fonts and
rendered nothing; fa-file-text-o maps to fa-file-alt in FA5 Free (there
is no fa-file-text glyph either).
